3-       PLEASE print Players name AND Parents name  & email address
4-       Checks or cash Only – no credit cards
5-       Make checks payable to : NW Lacrosse Boosters
6-       Put Payment & Form in an envelope and seal it.
7-       Give the envelope with form & payment to Coach Goldsmith
